  • The Gang That Couldn't Shoot Straight: Reconsidering the CDC Guideline

    After last year’s talk about the CDC Guideline, a lot has begun to change, begrudgingly. While this is encouraging, there are still both medical and nonmedical people who don’t want to see any changes in the weak to very weak (evidenced-based medicine) CDC Guideline, which has gone far from what it was supposed to be—recommendations for treatment in a primary care providers clinic. They have been “taken over, mandatorily” by a number of significant entities including CMS, various states, and more. Still, there appear to be political and nonpolitical but nonmedical mandated issues that continue to keep these “Guideline” in force. This presentation will discuss these and some possible reasons, both the positive and the negative, including important issues of forced tapering and increased suicides.

  • Pain Management at Ground Zero

    West Virginia continues to lead the nation, and world, in drug overdoses, which makes one ponder what is being done at the “ground zero” of the opioid epidemic to save and improve lives. Where better than where it’s worst should some of the possible solutions come from? In 2016, an interprofessional panel of experts in pain management—ranging from medicine, osteopathy, nursing, pharmacy, dentistry; public health; the state PDMP; and representatives from insurance providers—was put together with aims of finding possible solutions. The West Virginia Safe & Effective Management of Pain (SEMP) Guidelines (www.sempguidelines.org) were developed to facilitate the shift of the best practices in pain management becoming the new standard of care. The SEMP Guidelines include 2 main components including the risk reduction strategy and the clinical treatment algorithms. Pain management algorithms are not available anywhere else in the entire world! So we would like to welcome you to “the West Virginia Way” and see just how the “wild and wonderful” state of West Virginia is approaching the opioid epidemic from a true ground zero. After all, if it works where it’s worst, how could it not help your state or your practice?

  • Diabetic Peripheral Neuropathic Pain: Evaluating Treatment Options

    According to estimates from the Centers for Disease Control and Prevention,  29.1 million people (close to 10% of the US population) have diabetes and 30% to 50% of them eventually develop diabetic peripheral neuropathy, often with pain. Neuropathic pain due to diabetes is a commonly encountered chronic problem that has increased in frequency as the obese and diabetic population continues to increase. Many pharmacologic approaches including various algorithms are used to treat painful diabetic neuropathy; however, the effectiveness of many such medications is questionable. New evidence, including recent recommendations from the American Academy of Neurology, will be compared and contrasted with a prior systematic review and guideline, published in 2011. Agents and approaches with more recent evidence will be discussed in order to update knowledge of participants and assist them to better manage this disabling ailment.

  • Osteoarthritis Pain: Past, Present, and Future

    Pain related to osteoarthritis is extremely common, affecting a very large segment of the adult population. For years, many therapeutic approaches have been used with variable degrees of success. Over time, however, we have learned that some commonly used therapies may be ineffective or, worse yet, can lead to significant harm. In this course, available management methods, including oral agents, topical and physical agents, orthotics, and various injectable agents will be discussed, emphasizing existing and emerging medical evidence with proliferant/reparative agents and support of their use. Basic concepts regarding surgical options and their evidence will also be discussed in order for primary and specialty care providers in the audience to better use these treatment options, and know when to refer and how to educate their patients more effectively.
